Mobile Cellulitis Care
Cellulitis is a bacterial skin infection that requires prompt medical attention. While it is common, it can also become very serious over time. What starts with redness, swelling, and/or tenderness can eventually progress to more serious issues that require hospitalization. Seniors, people with diabetes, patients with chronic wounds, and those with compromised immune systems are particularly vulnerable and at risk when it comes to cellulitis.
At Heritage Wound Care, we provide mobile care for patients suffering from cellulitis. We offer bedside evaluation, treatment coordination, and ongoing monitoring services to ensure our patients receive the best care possible. In turn, our patients often avoid trips to the hospital.
Why Mobile Cellulitis Care Matters
Cellulitis can develop rapidly and, if untreated, may spread to deeper tissues, enter the bloodstream, and cause life-threatening infections. Access can be a major issue for patients in nursing facilities or rehab centers and those who are homebound, though. For these patients, traveling to an urgent care or outpatient clinic can be difficult. This is also the case for patients who are immobile, recovering from surgery, or managing chronic illnesses.
The mobile cellulitis care provided by Heritage Wound Care helps to address these issues and provides patients with the opportunity to receive the care they need and deserve. It also allows for early intervention, which can lower the risk of hospitalization.
Our Approach to Cellulitis Management
Our approach involves care performed within the homes or living facilities of our patients. This begins with an on-site assessment for cellulitis. We also review the patient’s history, current symptoms, and risk factors. From there, we offer diagnostic support, such as lab testing and/or imaging. This can be coordinated in partnership with facility staff or external service providers if necessary.
Once cellulitis is confirmed, provider orders are issued (often within twenty-four hours). This may include antibiotics, wound care support (when skin breakdown is present), and recommendations for elevation, hydration, or compression (when needed). We then follow up with weekly visits (or more frequent visits if required). During these visits, we monitor healing, adjust treatment, and take action to prevent complications.
Along with direct care, we educate patients, caregivers, and facility staff on what to look for. This helps with prevention and lowers the chance of repeated infections.
Collaborative Care Across Settings
The Heritage Wound Care model is designed to integrate seamlessly with the care provided by existing healthcare providers. We work with nursing facilities, rehab centers, and medical centers to ensure our patients receive comprehensive care without duplication. Along with nursing homes, rehab centers, caregivers, and primary care providers, we also work with hospitals and long-term acute care facilities. By working hand-in-hand with these providers, we are able to provide continuing care and reduce readmission.
This approach ensures that cellulitis care is not only effective but also coordinated across each stage of recovery.

Who Benefits from Mobile Cellulitis Care?
While cellulitis can affect anyone, certain patients are more vulnerable and benefit the most from the mobile care provided by Heritage Wound Care. These include:
- Seniors with fragile skin
- Patients with mobility challenges
- Patients with diabetes or poor circulation
- Those with chronic wounds or venous insufficiency
- Immunocompromised patients
- Post-surgical patients with incisions who are at risk of infection
For these patients, we offer an essential service and deliver timely evaluations and treatments without the need for travel or the risks that come with visiting hospitals.
Preventing Hospitalizations and Complications
When it comes to cellulitis, there is a high rate of hospital admissions. If left untreated, this ailment can lead to the formation of an abscess, sepsis, and even permanent tissue damage. It is extremely important to receive care for cellulitis. With timely mobile care, these risks are significantly reduced.
Heritage Wound Care provides early intervention and treatment. We offer on-site assessments, a quick turnaround of provider orders, and ongoing care. Our team focuses on treating cellulitis aggressively and stopping the issue from progressing.
Supporting Patients and Families
A diagnosis of cellulitis can be alarming, especially for patients with existing health conditions. Families also worry, as there is the potential for hospitalization or long-term complications. At Heritage Wound Care, we understand how scary infections can be for patients and loved ones. That’s why we communicate clearly and compassionately with patients and family members. We explain in detail what cellulitis is, how it is treated, and what to look for. By providing patients and their loved ones with clear and open communication, we are able to build trust and decrease the anxiety that comes with cellulitis treatment.
