Heritage Wound Care serves a variety of areas. Please reach out to learn more about coverage ( ). Our strategic coverage is specific to meeting the unique needs of referrals. When we receive a higher volume of referrals in any specific area, we immediately respond by expanding our team to meet those demands of our referral sources. We are committed to finding solutions, and if for some reason we are unable to cover a specific area, our intake team then communicates this transparently during the referral process. If we are actively marketing in that particular area, we are poised and ready to take on patients.
No. We will not duplicate the service of our home health partners. We invoice as a provider just like if a patient were receiving care from our provider at an outpatient wound care center.
We take most commercial insurance, Medicare, and Medicaid plans. Some insurance programs may require prior authorization, but we will notify you and your patient if that is requested.
Our providers fax their orders directly to your office within 24 hours of seeing your patient. We will also give you access to our system so your team can view our provider's orders and notes.
We usually see your patient around the same time each week. If we are scheduled to see a patient on a Tuesday, we will maintain that patient on the same schedule every Tuesday until their wound progresses (then seen every other week).
This alleviates the burden on your staff for transportation and safeguards your patient from unnecessary transfers that can promote pressure injuries and exacerbate wounds.
We do not order wound care supplies for your patient; however, we will put in our order for the most cost-efficient supplies and employ what is on the home health agency's formulary.
If you do not have a home health agency available, we will help get your supplies ordered, or if you need home health, we will also help establish that.
Everything Heritage Wound Care does to treat wounds in our outpatient wound care clinic, we can duplicate in the home. We utilize innovative treatments and technologies, from skin substitutes to state-of-the-art wound dressings, providing faster healing and improved patient outcomes.
